meshVerified COI Standards & Agency Governance Framework

Last updated: August 12, 2026

meshVerified™ is a product of Transharpe Innovations, Inc. This framework defines the standards, disclosures, and governance principles that shape how Certificates of Insurance are issued, verified, and interpreted within the meshVerified platform for transportation and logistics stakeholders.

The meshVerified Badge of Trust

The meshVerified Badge of Trust signals that a Certificate of Insurance has been reviewed against the meshVerified framework and is supported by machine-readable policy data, not merely a scanned PDF or manually keyed entry.

  • Badge eligibility criteria
  • Data freshness and re-verification windows
  • Revocation conditions and reissuance workflow
  • Visual treatment and placement in reports

Mandatory Standard of Care & ACORD Accuracy

Every COI reviewed under meshVerified must conform to the latest ACORD 25 standard. Coverage codes, limits, effective dates, and cancellation provisions must match the underlying policy declarations page to earn or retain verified status.

  • ACORD 25 field-by-field validation rules
  • Minimum required coverage limits by risk class
  • Policyholder vs. named insured matching
  • Cancellation notice and notice-of-cancellation handling

Explicit Contingent Labeling Requirements

Contingent or excess coverage must be labeled explicitly and never presented as primary liability. meshVerified requires clear, human-readable disclosures wherever contingent coverage is listed as part of a verification result.

  • Required contingent-coverage labels
  • Separation of primary and contingent rows
  • Tooltip and footnote disclosure language
  • Agency confirmation of contingent coverage scope

Sub-10,000 lbs. GVWR Exposure & Disclosures

Vehicles and equipment with a gross vehicle weight rating below 10,000 lbs. carry distinct risk profiles and regulatory treatment. meshVerified surfaces these disclosures so certificate holders can evaluate exposure accurately.

  • GVWR capture and validation rules
  • Lightweight-vehicle coverage considerations
  • Disclosure language for sub-10k units
  • Exclusion and exception handling

Direct Liability (FBL) vs. Contingent Policies

Freight brokers' liability policies are direct coverage for the broker's own operations, while motor carrier policies protect the transportation provider. meshVerified distinguishes these roles so stakeholders understand who is actually insured.

  • FBL policy identification rules
  • Primary vs. contingent motor carrier coverage
  • Named insured alignment with operational role
  • Cross-reference checks between broker and carrier COIs

Federal Filings (BMC-91X/MCS-90) Limitations

Federal filings such as BMC-91X and MCS-90 endorsements are required for for-hire interstate carriers but do not guarantee unlimited coverage. meshVerified displays filing status as informational context alongside the underlying policy.

  • BMC-91X and MCS-90 filing verification sources
  • Federal vs. state filing requirements
  • Limits of filing status as a coverage proxy
  • Hazardous materials and passenger-carrying exceptions

Advisory Board Governance & Crowdsourced Auditing

meshVerified maintains an independent advisory board of insurance, transportation, and compliance professionals. The board governs standards updates and reviews crowdsourced audit findings to preserve the integrity of the framework.

  • Advisory board membership and tenure
  • Standards revision and publication process
  • Crowdsourced audit submission workflow
  • Conflict-of-interest and recusal policies
